Here you will find a list of forms that are required by the Department of Social Services of Virginia. Most of these you will find can be downloaded with Microsoft Word.

4. Notice of Liability (this form falls under Juan's Law that was put into effect in July 2006 It states that any licensed/ register childcare provider is required to give you written notice whether the do or do not carry liability insurance)

5. Handbook and Handbook Statement

6. Physical Form and Updated Shot Records (.pdf)

7. Proof of Birth with viewing child's birth certificate or hospital letter within the first 7 days of care.

 

These forms are required by the start of care. Special circumstances can be taken under consideration and discussed during the interview process.
